NEW CLIENTS

All registration paperwork must be completed prior to intake appointment.

Upon scheduling your initial appointment the administrative staff will send you a portal invite via the email address you provide at registration, which will ask you to create an account. The invite will expire in two days so please create the account within that time frame. If your link expires, you will need to contact the office to have a new invite sent. Once your portal account has been established it can be accessed in the future by clicking the link to the left.
